Any student-athlete who is suspected of having head trauma which could be considered a concussion will be removed from participation until a proper medical evaluation can be performed. Students MUST be evaluated by a Student-athletes MUST be evaluated and cleared by a Licensed Health Care Providers (LHCP) which is required by the NCHSAA. The LHCP should have expertise and training in concussion management. LHCPs include the following individuals: Licensed Physician (MD/DO), Licensed Athletic Trainer (LAT), Licensed Physician Assistant (PA), Licensed Nurse Practitioner (NP), or Licensed Neuropsychologist.

All forms will be returned to the school's athletic trainer to clear the student-athlete for CHS participation regardless if its a CHS or Non-CHS injury. The school's athletic trainer will review all forms before the student-athlete is cleared to participate.
CONCUSSIONS AND ACADEMICS
Please understand that a student-athlete who has experienced a TBI or Concussion may have difficult in the classroom. Cognitive work may cause symptoms to worsen and therefore cognitive rest should occur. Basically the student-athlete should limit or alter their academic workload. Please consult a LHCP and CHS school counselor regarding classroom academic recommendations and a possible academic plan. The LHCP who evaluates and treats the athlete for a head injury (concussion) should complete a return to learn recommendations form and submit it to CHS councilors.
